We have created a special scheduling app just for you. It will make your clinic visits much smoother and hassle-free. You won't need to tell a scheduler and the receptionist and the patient care technician and the nurse and another nurse and (finally) the provider your story. You'll just tell us once.... right here.

When you create your appointment, you will chose a time block and your 'reason for visit'. It is important that you complete the reason for your visit very carefully. This ensures 3 things:

 

• We don't miss important details we need to get to the root of your issue

• You don't waste your time and energy

• You don't have to remember the details later

 

The provider will then confirm the purpose of your visit when you arrive, help you with your problem, and get you on your way sooner.

Once you have been registered in our database and filled out our intake forms, we will send you an email that will allow you to create a username and password to access secure patient information and a secure communication platform. You may then click on the link below and use your username and password to access the portal.
